During your appointment

Before your appointment:

  • If you have been experiencing any of the following symptoms in the last 7 days, PLEASE call us to reschedule your appointment. ​​

    • Cough, sore throat, fever, shortness of breath, diarrhea, loss of taste or smell​

  • All patients will be asked to review, fill out and sign our Self Declaration. You can also print this form and bring it with you. 

  • All New Patients will need to fill out their New Patient Paperwork online

  • Please wear a mask or buff to your appointment. 

  • To help limit the number of people in the office, we ask that whenever possible, patients do not bring children, other family members or friends with them to their appointment.

  • To limit the number of people in the waiting area, we will be utilizing a virtual waiting room. Patients will be asked to wait in their vehicle and will check in for their appointment by calling or texting the office at 970-476-3991. You will be notified when we are ready to begin your appointment.


When you arrive:

  • Please do not come inside. Instead please call the office at 970-476-3991 to let us know you have arrived. If you are unable to wait in your vehicle, you may come inside to check in. 

  • When we are ready to begin your appointment, the Front Desk will call to let you know you may come inside. Please leave backpacks and other large personal items in your car. Please wear your mask or buff.

  • When you come inside, the Front Desk will ask you to use hand sanitizer and will provide you with a garment bag to contain your jacket and any smaller personal belongings. 

  • If you did not bring the completed form with you, you will be asked to read and sign the COVID Self Declaration.


During your appointment:

  • Please wear your mask/buff before and after your visit begins

  • All patients will be asked to rinse a hydrogen peroxide mouth rinse, intended to reduce the viral load present in your mouth before beginning your any dental work.

  • You will notice that our team will be wearing more Personal Protective Equipment than they have in the past. Because asymptomatic patients can still carry the coronavirus, we need to take precautions with all appointments.

  • At the end of your appointment, we'll ask that you put your mask back on and use hand sanitizer before checking out.

  • Our payment devices are contactless capable and we encourage patients to use this feature if possible.
